Sleep medicine is a medical specialty or subspecialty devoted to the diagnosis and therapy of sleep disturbances and disorders. From the middle of the 20th century, research has provided increasing knowledge and answered many questions about sleep-wake functioning. The rapidly evolving field has become a recognized medical subspecialty in some countries. Dental sleep medicine also qualifies for board certification in some countries. Properly organized, minimum 12-month, postgraduate training programs are still being defined in the United States. In some countries, the sleep researchers and the doctors who treat patients may be the same people.

The first sleep clinics in the United States were established in the 1970s by interested doctors and technicians; the study, diagnosis and treatment of obstructive sleep apnea were their first tasks. As late as 1999, virtually any American doctor, with no specific training in sleep medicine, could open a sleep laboratory.

Disorders and disturbances of sleep are widespread and can have significant consequences for affected individuals as well as economic and other consequences for society. The US National Transportation Safety Board has, according to Dr. Charles Czeisler, discovered that the leading cause of fatal-to-the-driver heavy truck crashes is fatigue-related (fatigue – 31%, alcohol and other drug use – 29%), and sleep deprivation has been a significant factor in dramatic accidents, such as the Exxon Valdez oil spill, the nuclear incidents at Chernobyl and Three Mile Island and the explosion of the space shuttle Challenger.

What kind of sleep medicine is safe during pregnancy?
Q. I'm almost 30 weeks pregnanct and got maybe 5 hours of sleep last night. What can i take to get a good night's sleep, besides Tylenol PM. I just can't get comfortable. Is unisom or sleepytime (OTC medicines) ok?

A. I heard that sleepytime is ok, but I would check with your Dr. My Dr offered me Ambien's to sleep. I didn't take it up, as I already have a xanax prescription, and those knock me out. Good luck, and your almost there!

What over the counter medicine can help me sleep?
Q. I have been stress lately and I guess its started to affect my sleeping pattern. Any over the counter medication that you can recommend that can help me go to sleep? Thanks

A. I find that Melatonin (available at CVS & GNC) works wonders. I've been taking the lowest dosage possible for the past 3 years and it really helps you mellow out and get a good night's rest. Make sure you have at least 6.5-8 hours (everyone is different) devoted to sleep, otherwise you'll feel drowsy throughout the day.
